小儿脑性瘫痪中医文献计量学分析

【摘要】 目的 采用文献计量学方法对小儿脑性瘫痪(简称脑瘫)国内中医(含中西医结合)现代文献特征进行分析，以总结规律，指导临床及科研。方法 制定文献的纳入标准、排除标准以及文献检索策略，检索中国生物医学文献网络版数据库、中国知识网期刊全文数据库和重庆维普全文期刊数据库，检索时间为19940101/20080531，获得国内有关小儿脑瘫的相关文献;对15年间国内医学期刊正式发表的677篇小儿脑瘫中医(含中西医结合)文献进行文献数量、地区分布、研究方向、期刊分布的计量学研究。结果 小儿脑瘫中医现代文献数量总体呈上升趋势，河南、广东、江苏、湖南、上海、山东、辽宁、福建、北京、湖北等地区对其研究处于领先地位;主要以中医传统的针刺、推拿按摩、针灸及中西医结合康复方法为研究方向，但对早期干预及其他辅助治疗手段的研究仍需加强;文献集中发表于康复、针灸、神经疾病及中西医结合杂志。结论 小儿脑瘫国内的研究水平不断提高，已经形成热点研究地区及重点研究方向，但创新性有待进一步加强。

【Abstract】 Objective Use bibliometric methods to analyze the characteristics of TCM literatures(including the combination of TCM with Western medicine literatures) about cerebral palsy in children in order to summarize the regular patterns to guide clinical and scientific research.Methods Establish the inclusion criteria,exclusion criteria and search strategy of the literatures.Search modern literatures of cerebral palsy in children throughout the Online Chinese Biomedical Literature Database,CNKI and CQVIP from January 1,1994 to May 31,2008.Conduct bibliometric analysis on the quantity of the documents,geographical distributions,research orientations and periodical distributions based on the 677 domestic TCM literatures(including the combination of TCM with Western medicine literatures) published within 15 years in China.Results The number of modern TCM literatures showed a risen trend.Henan,Guangdong,Jiangsu,Hunan,Shanghai,Shandong,Liaoning,Fujian,Beijing,and Hubei led the development.The treatments had mainly based on TCM acupuncture and manipulation.The researches had mainly based on acupuncture and the combination of TCM with Western medicine rehabilitation methods.But studies on early interventions and other means of adjuvant therapy still needed to be strengthened.The documents had mainly been published in rehabilitation,acupuncture,neurological diseases and the combination of TCM with Western medicine journals.Conclusions The level of domestic studies on cerebral palsy in children are improving continuously.Hot research areas and key research orientations have emerged.But the innovations need to be improved.

小儿脑性瘫痪(简称脑瘫)是指自受孕开始至婴儿期非进行性脑损伤和发育缺陷所导致的综合征，主要表现为运动障碍及姿势异常[1]。归属中医“五迟、五软、五硬”范畴。脑瘫在有运动障碍的同时可伴有智力低下、惊厥发作、行为异常、听力障碍、视力障碍等，是儿童肢体残疾的主要疾病之一。随着围生期医学与新生儿抢救技术的发展，低体重儿及早产儿成活率的不断提高，小儿脑瘫的发病率呈上升趋势。
自1861年出现第1篇公开发表的脑瘫论文，脑瘫患儿的康复已成为世界各国科研机构研究的热点和难点。百余年来，现代康复医学对小儿脑瘫的定义、发病原因、诊断、分型、康复治疗、动物模型等方面的研究不断深入，取得了相当的研究成果;中国自20世纪80年代开始小儿脑瘫的研究，虽起步较晚，但对其中医病名规范、中医诊疗方案的梳理及中医康复治疗方面取得了长足的进步，特别是中医药结合传统中医康复手法，在减低肌张力，提高肌力，促进运动发育方面疗效显著。小儿脑瘫康复事业在迅速发展的同时，也不可避免地会存在一些不成熟的理念与实践。如何使国内小儿脑瘫康复事业沿着科学、规范、正确的轨道健康发展，己经成为全国儿童康复工作者共同关注的课题[2]。
近年来，国内关于小儿脑瘫的各类文献报道层出不穷，但国内小儿脑瘫的中医及中西医结合研究现状究竟如何呢?笔者从文献计量学角度分析近15年国内有关小儿脑瘫中医现代文献(含中西医结合)的特征和规律。现将研究情况总结如下。
1 资料与方法
1.1 文献纳入标准 同时符合以下条件的文献被纳入：(1)文献发表年限：19940101/20080531(共15年);(2)临床干预因素：中医疗法、中西医结合疗法;(3)以临床疗效观察为主要目的的汉语类文献。
1.2 文献排除标准 排除综述类、流行病学研究类、临床检验指标研究类、影像学研究类、并发症研究类、动物模型研究类及中国大陆地区以外的文献。
